HOW TO FILE A COMPLAINT

WITH THE AUDIT UNIT OF THE DIVISION OF WORKERS COMPENSATION

Below are the forms necessary to file a complaint and some very good information on the audit process.  They have been supplied by one of our Flight Attendants.  Click on the thumbnail (small images) to enlarge to readable size and print.   If you feel you have been abused by the Workers' Compensation or by Liberty Mutual please complete the forms and send them in.  The forms are the California Workers' Compensation Audit forms but can be found and downloaded from every state.

Below is a response from the Audit Unit to the Flight Attendant.  I have omitted the name of the Flight Attendant for privacy reasons but feel the information is very useful.  Especially the line stating "If you do not have legal counsel, additional assistance may be available through the Information and Assistance office at the Los Angeles Workers' Compensation Appeals Board (WCAB).
